Job Description
A forward-thinking telehealth practice is seeking a Remote Nurse Practitioner (NP) to deliver chronic care and lifestyle medicine via virtual consultations. Providers will manage patient health through data-driven care, collaborate with a multidisciplinary team, and empower patients with wellness education. Key Responsibilities: • Conduct telehealth visits and develop individualized care plans • Review patient data from EMRs and wearable devices to guide treatment • Monitor progress, adjust care plans, and educate patients on wellness and prevention • Collaborate with lifestyle educators and data scientists for holistic care • Document encounters accurately in EMR systems Qualifications: • Active NP license (CA, FL, or TX) • FNP or AGACNP preferred • Experience in chronic care or telehealth (new grads with interest welcome) • Strong communication and data analysis skills • Passion for patient-centered, holistic care Benefits & Compensation: • $65/hour • Flexible, remote scheduling • Collaborative and supportive care model • Growth opportunities in lifestyle and data-driven medicine Company DescriptionNPHire is the go-to job board for Nurse Practitioners.
