Job Description: The OASIS Reviewer Home Health (REMOTE ROLE) is responsible for ensuring the accuracy, compliance, and quality of all OASIS assessments within a Home Health agency. This role supports clinicians by reviewing submitted documentation, identifying inconsistencies, and ensuring alignment with CMS regulations, agency policies, PDGM requirements, and all federal/state guidelines. The OASIS Reviewer plays a vital role in improving patient outcomes, reimbursement accuracy, and overall documentation integrity. Key Responsibilities: β’ Review and audit Start of Care (SOC), Recertifications, Resumptions of Care, Transfers, and Discharges for accuracy and regulatory compliance. β’ Ensure all OASIS assessments align with clinical findings, visit notes, physician orders, and plan of care. β’ Identify documentation gaps and provide feedback or clarification requests to field clinicians. β’ Collaborate with QA, clinical managers, and field staff to improve the quality and consistency of documentation. β’ Assist with correcting or updating OASIS data elements to support accurate coding and PDGM scoring. β’ Monitor OASIS submission deadlines and ensure timely submission to CMS. β’ Maintain up-to-date knowledge of OASIS-E guidance, CMS Q&As, COPs, PDGM rules, and regulatory changes. β’ Review functional scoring, M-items, and diagnosis coding for internal accuracy and compliance. β’ Support training of clinicians on best practices for documentation and OASIS completion. β’ Participate in QAPI activities, quality audits, and performance-improvement initiatives. β’ Communicate effectively with interdisciplinary team members to resolve discrepancies and improve documentation standards. Qualifications β’ Must have 12 months of recent experience within the last 3 years. β’ Strong proficiency with Home Care Home Base (HCHB) or other EMR systems. β’ BLS from AHA highly preferred β’ Excellent communication and interpersonal skills for effective patient and family education. β’ Current and unencumbered Registered Nurse (RN) license in the state of California. β’ Reliable transportation and a valid driverβs license for travel between patient locations.
