Role Description
- This role involves providing virtual patient care through chronic care management.
- Conduct thorough assessment calls with patients to gather relevant information about their health status, concerns, and needs
- Identify and escalate patient needs, including refill requests, appointment scheduling, and connecting patients with community resources
- Provide patients with relevant and clear information about their health conditions, treatment plans, and self-management strategies
- Collaborate with patients to establish realistic and achievable health goals, aligning with their overall care plans
- Qualifications
- Active LVN/LPN license (required)
- At least 1 year of nursing experience preferred (RPM, telehealth, or chronic care experience a plus)
- Strong communication skills with the ability to build patient trust remotely
- Proficient with computers (Mac), EMRs, and telehealth tools
- Reliable, high-speed internet and a HIPAA-compliant home workspace
- A hardwired internet connection is required to ensure call clarity
- Requirements
- Schedule: Monday - Friday 9:00 am - 6:00 pm EST
