## About This Role
Magenta Care is seeking a highly organized, proactive Care Coordinator with a Medical Assistant background to support our growing home health and home infusion operations. This role is central to ensuring smooth patient intake, timely scheduling, strong communication, and seamless coordination between patients, nurses, providers, and internal leadership.
As the primary point of contact during business hours, you will serve as the communication hub for Magenta Care — fielding calls from patients, families, and providers, coordinating schedules with nursing staff, and ensuring that documentation and processes run efficiently behind the scenes.
The ideal candidate is detail-oriented, adaptable, and comfortable working in a fast-paced, evolving environment. This role requires strong administrative skills, healthcare knowledge, and the ability to manage multiple priorities with professionalism and compassion.

## Schedule & Availability
This is a full-time position with scheduled hours of Monday through Friday, 8:30 AM to 5:00 PM PT, including a 30-minute unpaid lunch break (40 paid hours per week). The Care Coordinator provides primary phone coverage during these hours.
Home health coordination occasionally requires communication outside standard business hours — such as nurse call-outs, schedule changes, or urgent patient needs. All hours worked, including after-hours communication, are tracked and compensated. Calls received outside scheduled hours are routed to the Director of Clinical Services or voicemail; the Care Coordinator reviews and follows up during the next business day.

## Key Responsibilities
### Phone Coverage & Communication
1. Serve as the primary phone contact for Magenta Care during business hours (8:30 AM – 5:00 PM PT)
1. Answer incoming calls from patients, families, referral sources, and providers
1. Communicate clearly, professionally, and compassionately with all parties
1. Serve as the key communication bridge between patients, nurses, providers, and leadership
1. Escalate clinical or operational concerns to the Director of Clinical Services as appropriate

### Patient Intake & Coordination
1. Receive and process new patient referrals
1. Collect and verify patient demographics, insurance information, and required documentation
1. Communicate with patients and families regarding next steps, scheduling, and expectations
1. Coordinate start-of-care visits and ongoing services

### Scheduling & Nurse Coordination
1. Schedule home health and home infusion visits
1. Communicate assignments and updates to nursing staff
1. Adjust schedules as needed due to patient or nurse availability
1. Support coverage coordination and last-minute changes

### Administrative & Operational Support
1. Maintain accurate and up-to-date patient records in the EMR
1. Track authorizations, orders, and required documents
1. Support leadership with operational tasks as needed
1. Assist with internal process improvement and workflow organization

### Communication & Collaboration
1. Serve as a key point of contact between patients, nurses, providers, and leadership
1. Communicate clearly, professionally, and compassionately
1. Escalate clinical or operational concerns appropriately

    ### Compliance & Quality Support1. Ensure documentation completeness and accuracy1. Support compliance with agency policies and regulatory requirements1. Assist with chart preparation and audits as directed

Requirements: Certified & experienced Medical Assistant. Prior experience in healthcare coordination, clinic operations, home health, or similar settings. Strong organizational and time-management skills. Comfortable using EMRs, scheduling systems, and basic office technology. Excellent written and verbal communication skills. Ability to work independently and manage shifting priorities
