Overview
We are seeking a high-level grant writer with strong healthcare and rural health experience to support development of a competitive application for the South Carolina Rural Health Transformation Program (RHTP).
This is not a blank-page project. We have already developed substantial program content, a clear service model, support letters, staffing structure, and a defined rural footprint. We are looking for a writer who can elevate, organize, and strategically package the information into a polished, funder-aligned application.
We want someone who understands how to position care transformation, chronic care, case management, access, workforce, and sustainability in a way that is compelling to reviewers.
About the Project
Our model combines:
PC360 Health as the parent clinical platform
360 Case Management as the care-coordination partner
The project serves rural South Carolina counties including:
Aiken
Barnwell
Allendale
Edgefield
McCormick
Hampton
Jasper
The model includes:
advanced primary care
chronic care management
transitional care management
case management
SMART-goal health coaching for clients with chronic conditions
community-based referral and support partnerships
workforce development and care-team scaling
Current Organizational Readiness
We are not starting from scratch.
360 Case Management has been in operation since November 2019
PC360 Health / 360 Chronic Care Management has been developing since 2023
The clinical model has gone live on a small scale to test workflows, refine systems, and strengthen readiness for scale
We have been preparing for this grant application since November 2025
We have partner relationships in development with community-based providers, including home care organizations
We have support letters and core narrative materials already assembled
Care Team Structure
Our model includes:
National Board Certified Health and Wellness Coach
Collaborating physician
3 Nurse Practitioners
2 MSWs
1 RN CCM to review assessments
3 RNs for Transitional Care Management
1 CNA serving as practice manager
Funding Opportunity Timeline

Grant deadline: June 1, 2026
We want to engage a writer immediately so strategy and drafting can be completed well before the deadline.
Scope of Work
We are looking for support with:
reviewing the grant opportunity and application instructions
refining and drafting the 1-page project summary
refining and drafting the 10-page project narrative
strengthening the organizational background
assisting with the budget narrative
tightening alignment with the selected funding initiative
helping present our model in a clear, competitive, reviewer-friendly format
final editing and packaging support
What We Need in a Strong Candidate
We are looking for someone with demonstrated experience in one or more of the following:
healthcare grant writing
rural health funding
Medicaid or state healthcare transformation grants
HRSA, CMS, state health agency, or similar public-sector funding
primary care, chronic care, workforce, RPM, or care coordination program design
The ideal writer will be able to quickly understand our model and help us strengthen:
project framing
measurable outcomes
sustainability language
rural health need statement
program alignment with state priorities
