Float Clinical Care Coordinator (RN MDS / Assessment Coordinator – RNAC)
Location: Harrisburg / Lancaster, PA (and surrounding areas)
Employment Type: Full-Time | Flexible Schedule (Remote Flexibility)
Travel Requirement: Travel between client facilities required (mileage reimbursement provided)
Position Overview
We are seeking an experienced Float Clinical Care Coordinator (RN MDS / RNAC) to support nursing homes throughout the Harrisburg and Lancaster regions. This role assists, substitutes for, and trains MDS Coordinators as needed across multiple facilities.

Primary Responsibilities
- Clinical Support & Training
- Support nursing facilities and MDS Coordinators with accurate completion of resident assessments in accordance with state and facility policies
- Conduct clinical record reviews to ensure consistent, accurate documentation
- Train and mentor MDS Coordinators on assessment processes and documentation standards
- Provide hands-on clinical support tailored to individual facility needs
- Intervention & Auditing
- Identify areas of concern and collaborate with facility leadership to develop effective action plans
- Conduct audits, interventions, and follow-ups based on facility-specific requirements
- Remote & On-Site Flexibility
- Semi-remote work may be available for candidates with 5+ years of MDS experience and Pennsylvania Case-Mix experience
- Qualifications
- Active Registered Nurse (RN) license in Pennsylvania
- Minimum 1 year of experience as an RN Assessment Coordinator (RNAC) with MDS expertise
- Strong understanding of the MDS process and regulatory requirements
- Ability and flexibility to float between multiple facilities
- Strong communication, organizational, and clinical training skills
