Telemedicine Internal Medicine / Family Medicine Physician — Clinical Oversight & Credentialing
Role Overview
We are seeking a nationally-licensed, board-certified Internal Medicine or Family Medicine physician to serve as the primary telehealth provider for CareSola's chronic care navigation programs. This is a part-time, remote role focused on clinical oversight, care plan approval, and serving as the credentialed provider on our Medicare Advantage and Medicare Part B payer contracts. We are specifically looking for a physician whose payer-facing credentialing profile is cleanly aligned with primary care / outpatient medicine.
This physician will be the clinical backbone of our care navigation operation — reviewing and signing off on care plans developed by our care navigators, ensuring clinical compliance, and serving as the credentialed provider that allows CareSola to bill Medicare for CHI, PIN, CCM, and RPM services.
About CareSola
CareSola is a chronic care navigation company delivering Community Health Integration (CHI), Principal Illness Navigation (PIN), Chronic Care Management (CCM), and Remote Patient Monitoring (RPM) services to Medicare beneficiaries. We operate under an MSO-PC model with professional corporations in Florida, New Jersey, and California (with additional states in formation). Our parent company, AgentOffers, also owns Life Shield Medical, a personal emergency response systems (PERS) business — 95% of whose customers have a chronic condition, creating a strong cross-sell pipeline into care navigation.
- Key Responsibilities
- Review and approve care plans for CHI (G0019, G0022) and PIN (G0023, G0024) services
- Provide clinical oversight and sign-off on care navigator activities
- Serve as the credentialed provider on Medicare Advantage and Medicare Part B payer contracts
- Participate in quarterly compliance meetings with the MSO
- Maintain active, unrestricted medical licensure in all 50 U.S. states (required)
- Collaborate with the operations team to ensure clinical protocols meet CMS requirements
- Support credentialing and enrollment processes with Medicare Advantage plans
Requirements
- Must-Have:
- M.D. or D.O. with active, unrestricted medical licenses in all 50 U.S. states
- Board certification in Internal Medicine or Family Medicine; active outpatient / primary care orientation required
- NPPES and CAQH profiles must read cleanly as primary care / outpatient medicine
- No Emergency Medicine, Hospital Medicine / Hospitalist, Pathology, or Anesthesiology taxonomy overlap on NPPES or CAQH
- No current or prior Medicare Advantage credentialing under Emergency Medicine or Hospital Medicine / Hospitalist specialty
- Active NPI in good standing with no sanctions or exclusions
- Eligible for credentialing with Medicare Advantage payers as a primary care / outpatient provider without expected specialty exceptions or remediation
- Experience with Medicare billing and compliance requirements
- Active, unrestricted medical licensure in all 50 U.S. states required
- Preferred:
- Experience in virtual / telehealth care delivery
- Familiarity with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or care navigation programs
- Experience with MSO-PC structures or value-based care models
- Prior Medicare Advantage credentialing as Internal Medicine or Family Medicine is strongly preferred
- Clinical leadership experience (quality improvement, care protocol development)
Compensation
- This is a full-time, remote (telehealth) role. Compensation is competitive and commensurate with experience.
- Per-visit compensation: $20/visit
- Expected volume: 50 – 100 visits/day
- Estimated monthly earnings: >$20,000 (based on ~20 working days)
- Structured as an independent contractor or employed through the professional corporation, depending on preference
- Malpractice insurance coverage provided by the PC
Engagement Structure
The physician will be engaged through one of CareSola's state-level professional corporations (CareSola Medical Services PA, East PC, or West PC) under our MSO-PC model. The MSO handles all non-clinical operations — technology, marketing, patient acquisition, billing, compliance infrastructure, and care navigator management. The physician's role is focused exclusively on clinical oversight and care plan approval.
Pay: $200,000.00 - $280,000.00 per year
