The Home Care Physical Therapist is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision-making skills. The Home Care Physical Therapist cares for a caseload of home care patients by evaluating the patient for appropriateness of home care needs and developing the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care Physical Therapist provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Home Care Physical Therapist provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care Physical Therapist documents in the patient’s medical record in a timely manner according to agency policy. The Home Care Physical Therapist attends and actively participates in Case Conference and team meetings. The Home Care Physical Therapist performs on-call responsibilities. The Home Care Physical Therapist supervises and delegates appropriate functions to the Physical Therapist Assistants. Required Education: Master’s degree in Physical Therapy Preferred Education: Doctorate degree in Physical Therapy Required Skills: Therapy skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously. Preferred Skills: Experience with Home Care Home Base; Experience with smart phones and smart devices Required Licensure/Registration/Certification: Current Texas license as an Occupational Therapist; CPR; Current driver’s license and auto insurance. Must have reliable transportation Preferred Licensure/Registration/Certification: Advanced skills (or) certifications in specialty areas, such as NDT, geriatric, orthopedics, lymphedema etc. Required Experience: Work (or) educational experience in the healthcare field Preferred Experience: One year experience as a Physical Therapist in a health care setting
