About the position

Mindful Care is seeking a dual licensed (CT & NY) Clinical Intake Specialist to provide intake services at our Stamford, CT office. Our Clinical Intake Specialists (LMSWs) play a critical role in a patient's first experience with Mindful Care. You'll lead comprehensive initial evaluations, helping patients feel seen, understood, and supported from day one. Using a biopsychosocial lens, you'll assess needs, identify appropriate services, and collaborate with the broader clinical team to build thoughtful, effective treatment plans. This is a high-impact role for clinicians who are confident in their assessments, grounded in clinical knowledge, and genuinely care about patient outcomes.

    Responsibilities
  • Conduct initial psychiatric evaluations (90791) for all new patients
  • Complete thorough biopsychosocial assessments and mental status exams
  • Utilize screening tools including PHQ-9 and GAD-7 to guide clinical decision-making
  • Develop and recommend individualized treatment plans based on patient needs
  • Act as a subject matter expert on Mindful Care services and care pathways
  • Collaborate closely with psychiatrists, therapists, and clinical leadership
  • Participate in clinical meetings, trainings, and case discussions
  • Provide support to the Therapy team as needed
  • Maintain strict adherence to ethical and clinical standards
  • Deliver a consistently patient-centered, compassionate experience
    Requirements
  • Master's degree in Social Work (Required)
  • Active LMSW license in both New York and Connecticut (Required)
  • Strong assessment skills, including mental status exams and clinical screenings
  • Hands-on experience with PHQ-9 and GAD-7 (this is a must)
  • Solid understanding of DSM-5 diagnoses
  • Excellent communication and interpersonal skills
  • Strong organization and time management
  • Team-oriented mindset with a collaborative approach
  • Passion for working with diverse populations (ages 12 through adulthood)
