Experience: • Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
• Basic computer skills and proficiency with bolthires Office products. • Strong organizational skills, interpersonal skills, and detail-oriented. • Experience maintaining focus and meeting goals in a work-from-home environment. • Previous call center experience Pharmacist - Medication Therapy Management (MTM) Overview: • Medication Therapy Management supports the company vision to help people on their path to better health by performing MTM for members at the highest risk of having medication-related problems • The team of pharmacists, technicians, and coordinators works together to identify problems and coordinate care to facilitate resolution to ensure each member is on an optimized medication regimen to help them reach their health-related goals.

Duties: • Perform telephonic comprehensive medication reviews, including medication reconciliation and clinical assessments for targeted members • Identify, document, and help resolve medication-related problems • Create written patient summaries at an appropriate health-literacy level • Prepare faxed materials for prescriber offices to resolve medication-related concerns • Build and maintain positive relationships with internal and external partners, such as physicians and nurses • Perform other responsibilities as directed by supervisor or manager Estimated Start Date: • 01/12/2026 Specific Details to Call Attention To: • Must reside and hold an active pharmacist license in HI, TX, IL, FL, or PA • Strong focus on telephonic MTM delivery and documentation • Work-from-home environment requires strong focus, independence, and reliability • * Only those lawfully authorized to work in the designated country associated with the position will be considered.
