Judge Healthcare is currently seeking Service Coordinator Clinicians around Nashville, Knoxville and Memphis, TN!
Job Details: Ongoing Contract (W2, Weekly Pay, Benefits Available)
Location: Local travel around the above locations, TN and working from home (Mileage Reimbursed for local travel!)
Hours: Monday-Friday 8AM-5PM CST
Compensation: Competitive and based on experience
Responsibilities:
Assist telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports.
Assists responsible RN in identifying members for high risk complications and obtain clinical data
Identify members that would benefit from an alternative level of care or other waiver programs.
Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost effective and efficient utilization of health benefits
Qualifications & Requirements:
Requires LPN or LCSW licensure in the state of TN
Experience in hospital setting, Home Health setting or case management
2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities
