Job Summary:
We are seeking a dedicated Medical Assistant to join our team on a part-time basis. This role involves conducting Chronic Care Management (CCM) calls and assisting in patient management. The ideal candidate will be compassionate, detail-oriented, and driven by a passion for transforming healthcare through technology.
- Responsibilities:
- Conduct CCM calls to patients, following up on their health status and care plans.
- Maintain accurate and detailed clinical documentation in electronic health systems.
- Assist in scheduling and confirming appointments as needed.
- Collaborate with healthcare providers to ensure coordinated and comprehensive care.
- Provide information to patients regarding their health conditions and care management plans.
- Ensure compliance with all healthcare regulations and privacy laws.
- Qualifications:
- Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) preferred.
- Experience in Chronic Care Management or similar roles is highly desirable.
- Strong interpersonal and communication skills.
- Proficiency in using healthcare management software and electronic health records.
- Ability to work independently and as part of a team.
- Passionate about using technology to improve healthcare delivery.
