Waypoint Care Management is hiring experienced Care Managers to serve as concierge guides for families navigating their aging loved ones' healthcare needs. This is not a hospital job. Not an insurance job. It is the role you did not know existed — built entirely around your clinical expertise, your schedule, and your patients.
What's more, YOU choose how much you want to work! Are you interested in some additional income and only have time for one client, great! Or, you're looking for full time work and want to take on 10 or more clients - also great! We're here for you either way.

WHAT YOU WILL ACTUALLY DO
━━━━━━━━━━━━━━━━━━━
- Serve as the trusted, boots-on-the-ground guide for families who cannot be there themselves
- Conduct comprehensive assessments and develop individualized care plans
- Coordinate care across physicians, specialists, and care facilities
- Advocate fiercely for your clients' dignity, preferences, and clinical outcomes
- Communicate proactively with families — clinical updates, real answers, vivid reporting

WHAT YOU WILL NOT DO

- Chase down invoices — we handle all billing and collections
- Beg for referrals — we fund and run all marketing and lead generation
- Drown in admin — our technology stack automates it
- Hit patient volume quotas — quality over quantity, always

THE NUMBERS
━━━━━━━━━━━━━━━━━━━
You earn 60% of each client's monthly retainer — with a path to 70% as your caseload grows. W-2 payroll via Gusto, paid bi-monthly. Client monthly retainer's range from $1,500 - $2,500+

WHO WE ARE LOOKING FOR
━━━━━━━━━━━━━━━━━━━
- Registered Nurse (RN), Licensed Social Worker (MSW/LCSW), Discharge Planner, or Case Manager
- Minimum 3 years of clinical or care coordination experience
- CMC certification (active or actively pursuing) through NACCM preferred
- Exceptional communication skills — you are the calm in someone else's storm
- Phoenix/Scottsdale area presence for in-person client visits
