Case Manager - Healthcare Consultant Estimated Length of Assignment: 03+ Months with Possible Extension Pay Range: $30.00/Hour to $34.13/Hour (W2, All Inclusive) Work Type: 50‑75% travel for face‑to‑face visits with members and 25‑50% work from home Schedule: Monday – Friday 8:00 AM – 5:00 PM Location:
Remote (work from home). Candidates must reside in the Miami, FL area (Miami, North Miami, Aventura, Hialeah, Miramar). Travel is required approximately 75% of the time within the region to visit members at home, assisted‑living facilities, and nursing homes.
Responsibilities • Coordinate case‑management activities for Medicaid Long Term Care/
Comprehensive Program enrollees. • Conduct comprehensive evaluation of members using care‑management tools and data reviews. • Implement and monitor assigned care plan activities. • Perform multidisciplinary reviews to achieve optimal outcomes. • Identify and address quality of care issues through established channels. • Use negotiation skills to secure appropriate options and services for members. • Apply motivational interviewing to encourage member engagement and lifestyle changes.
• Coach members to make independent medical and healthy lifestyle decisions. • Collaborate with providers in healthcare decision‑making and document care processes. Qualifications • Ability to multitask, prioritize, and adapt to a fast‑paced, changing environment. • Strong verbal and written communication skills. • Details oriented, highly organized, and tech‑savvy. Preferred Experience • Long‑term care experience (preferred). • Bachelor’s degree required (Social Work or related field). No nursing degrees acceptable.
