For a successful candidate, this is a long term project. JOB DESCRIPTION The job seeker will need to be a Registered Nurse with case management experience. The successful RN will coordinate, screen, and assesses the home health care needs of patients referred to the Agency. SKILLS β’ Reviews incoming physicians orders and other clinical documents β’ Collaborates with physicians, nurses, and discharge planners to identify patient care needs and to establish the appropriate level of care for patients referred to the Agency β’ Ensures that the needed DME, medical supplies and ancillary services are ordered and documented β’ Demonstrates a high degree of commitment to customer relations and quality service β’ Utilizes community resources as appropriate to meet patient care needs We look forward to hearing from you!
