Job Description
Howard Memorial is in need of an RN Case Manager Director. This position will provide case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
This is a Part-Time job status position.
A minimum of 3 years clinical nursing experience is preferred for this position.
Responsibilities
Ā Provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs
Ā Coordinate the integration of social services/case management functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and healthcare facilities
Ā Communicate with physicians at regular intervals throughout hospitalization and develops an effective care plan to maintain appropriate cost, case and desired patient outcomes
Ā Facilitates interdisciplinary patient care conferences to review treatment goals, optimize resource utilization, and identified post-hospital needs
Ā Assess patient progress throughout hospital stay, while serving as patient and family advocate
Ā Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals
Ā Supervises the duties and schedule of the Utilization Review Nurse
Qualifications
Ā Current Arkansas or Compact State RN licensure
Ā Graduate of board approved nursing program
Ā ASN required, BSN preferred
Ā Effective communicator with critical thinking skills
Ā Readily exhibits leaderships skills
Ā BLS, ACLS certifications
Obtain Case Manager certification within 5 years of entry into position.
