This position is responsible for member-facing and provider-facing outreach to support Medicare Part D stars initiatives including any current Part D measures. Calls are made to members on the West coast, so work hours will be during appropriate call times for the Pacific Time (PT) zone. • Execute clinical pharmacy programs • Coordinate strategies for increasing adherence • Utilize motivational interviewing and health coaching to impact member behavior • Outreach to members 90% of the day • Ensure effective collaboration with internal and external matrix partners to support clinical programs • Work collaboratively with leadership and management to strengthen business and operational processes • Other duties as required Qualifications • 1+ years of experience with Medicare Stars, managed care or population health • 2+ years of customer or patient-facing experience • Enhanced knowledge of pharmacy and claims processing • Excellent organizational and analytical skills • Superior communication skills with patients and clinicians • Ability to prioritize daily to monthly deliverables • Computer Skills: Proficient in bolthires Excel, bolthires Teams, and Sharepoint Requirements • Required proficiency in bolthires Excel, Teams and Sharepoint • This position could involve travel up to 10% Preferred Qualifications • Certified Pharmacy Technician (CPhT), Technician Trainee license, Licensed Practical Nurse (LPN), Certified Medical Assistant (CMA), or equivalent clinical training • Knowledge of formulary designs, prescription benefit trends • Bilingual in English and Spanish Education • High School Diploma, Associates degree preferred Benefits • Affordable medical plan options • 401(k) plan (including matching company contributions) • Employee stock purchase plan • No-bolthires programs for all colleagues including wellness screenings, tobacco cessation and weight management programs, confidential counseling and financial coaching • Benefit solutions that address the different needs and preferences of our colleagues including paid time off, flexible work schedules, family leave, dependent care resources, colleague assistance programs, tuition assistance, retiree medical access and many other benefits depending on eligibility Apply tot his job
