We are seeking a dedicated and experienced Hospice Patient Care Coordinator to join our team at Intrepid USA, a leading national home healthcare company. As a Hospice Patient Care Coordinator, you will play a vital role in providing clinical and management support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, patient-centered care to our hospice patients and their families.


Key Responsibilities:



  • Provide clinical and management support to the Clinical Nursing Supervisor to ensure quality and safe delivery of hospice care services

  • Assist with staffing and scheduling, admissions, and nursing visits as needed

  • Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers

  • Assure efficient coordination of caregivers through case conferences and scheduling processes

  • Monitor patient records to ensure compliance with documentation requirements and program-specific rules

  • Participate in quality assurance and performance improvement activities

  • Provide orientation, training, and in-services to new hires and the care team

Requirements:



  • Graduate of an accredited school of nursing

  • Current Registered Nurse licensure (in good standing) in the state(s) of practice

  • Current CPR certification

  • Valid driver's license and automobile insurance

  • Three (3) years of clinical experience, including two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting
