About the position

    Responsibilities
  • Ensure continuity of care for consumers during the evening shift.
  • Develop and maintain relationships with staff and families.
  • Act as a liaison between Support Coordinating Organizations, Behavior Specialists, and external stakeholders.
  • Familiarize with Charting the LifeCourse and various support programs.
  • Manage clock-ins and clock-outs for staff.
  • Schedule and reach out to families regarding attendance issues.
  • Contact staff about attendance concerns.
  • Coordinate between Recruitment and Program Coordination departments.
  • Hold weekly information sessions in the evenings.
  • Review progress notes and provide staff updates.
  • Lead DSP meetings to discuss progress and concerns.
  • Reach out to families for staffing updates and follow up on applications.
  • Conduct monthly virtual wellness checks with families and individuals.
  • Provide progress regarding supports and obtain feedback from families.
  • Conduct monthly progress check-ins with staff.
    Requirements
  • High school diploma and three to five years of related experience required.
  • Bachelor's degree preferred.
  • Must pass a criminal background check and health screening.
  • Maintain a valid driver's license.
  • Good communication skills (verbal and written).
  • Interpersonal and organizational skills required.
  • Ability to monitor and assess personal performance and that of others.
  • Knowledge of the healthcare industry as it relates to support qualifications.
  • Experience with caseload management, typing, reading, and writing.
  • Ability to motivate staff and resolve tasks positively.
    Nice-to-haves
  • Experience in scheduling (1 year preferred).
  • Experience managing caseloads (1 year preferred).
  • Experience with IDS Waiver & ODP (1 year preferred).
  • Experience developing relationships with families (1 year preferred).
  • Experience working directly with staff (1 year preferred).
  • Experience training staff (1 year preferred).
  • Experience working with adults with special needs (1 year preferred).
  • Experience in a startup company (1 year preferred).
  • Experience with virtual communication and professional emails (1 year preferred).
  • Experience making and receiving inbound and outbound calls (1 year preferred).
