Wholistic Wound Care & Wellness is expanding and seeking a Texas Licensed Nurse Practitioner to provide primary care and wellness-focused services in our San Antonio clinic. This role is ideal for a provider who enjoys preventive medicine, lifestyle counseling, and long-term patient relationships while managing common primary care conditions.

- What You’ll Do
- Provide comprehensive primary care visits and patient evaluations
- Conduct annual wellness visits and preventive health screenings
- Manage chronic diseases such as diabetes, hypertension, and hyperlipidemia
- Order and interpret laboratory and diagnostic tests
- Prescribe medications and adjust treatment plans
- Develop personalized wellness and lifestyle plans
- Educate patients on nutrition, weight management, and preventive health
- Coordinate care with specialists and other providers when needed
- Maintain accurate and timely clinical documentation
- What We’re Looking For
- Licensed Nurse Practitioner in Texas
- Board Certified (FNP, AGNP, or Adult NP)
- Active National Provider Identifier (NPI)
- DEA registration preferred
- Experience in primary care, family medicine, or internal medicine preferred
- Strong communication and patient education skills
