Responsibilities include: Perform in-home physical therapy evaluations and treatments Develop and implement individualized care plans Complete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges Evaluations Educate patients and caregivers on mobility, safety, and recovery Collaborate with physicians and interdisciplinary care teams Supervise Physical Therapy Assistants (PTAs) when applicable Complete documentation using AI-assisted EMR tools Qualifications Active, unencumbered Florida Physical Therapist license OASIS documentation experience required CPR certification Valid driver’s license, reliable transportation, and auto insurance Professional liability insurance Workers’ Comp Exemption or Occupational Accident coverage Lymphedema certification preferred Join a Growing Home Health Leader If you're ready to lead a clinical team, drive quality care, and grow your leadership career , we’d love to connect.
