pstrongWhat You'll Do as a Hospice RN / RN Case Manager:/strong/p

liBe the primary point of contact for patients and families, guiding them through every step of their hospice journey./li
liDeliver and document skilled, hands-on nursing care based on each patient’s individualized strongPlan of Care/strong—in their home, assisted living, or wherever they call home./li
liPerform stronginitial and ongoing assessments/strong to monitor patient condition and adjust care as needed./li
liCollaborate with an stronginterdisciplinary team/strong—including physicians, LPNs, CNAs, social workers, chaplains, and others—to build and refine personalized care plans./li
liProvide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well—offering education, comfort, and bereavement guidance./li
liTeach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance./li
liCommunicate important updates in patient condition to the care team, ensuring timely and appropriate interventions./li
liMaintain accurate, up-to-date strongclinical documentation/strong and follow infection control and compliance standards./li
liSupport the organization's strongquality improvement/strong and scheduling initiatives to ensure care is consistent, compassionate, and efficient./li
liHelp patients and families understand and navigate topics such as:/li

liMedication administration/li
liHospice philosophy and services/li
liSymptom and pain management/li
liEnd-of-life processes and expectations/li

pstrongQualifications – What You’ll Bring:/strong/p

listrongActive RN license/strong in the state of employment (or eligible to obtain)./li
listrong1+ year of RN experience/strong in hospice, home health, ICU, oncology, geriatrics, or related nursing fields./li
liA genuine stronghospice heart/strong: compassionate, empathetic, and patient-centered./li
liComfortable providing care in diverse settings, including private homes and facilities./li
liStrong communication and critical thinking skills in emotionally sensitive situations./li
liValid strongdriver’s license/strong, auto insurance, and reliable transportation./li
listrongCPR certification/strong required./li
