The Role...

This role is 70% in person, 30% remote

Are you a compassionate and dependable registered nurse looking to make a meaningful difference? Join our home health care team in the Fort Collins, CO area as an RN Case Manager. In this role, you'll play a crucial part in developing personalized care plans and delivering skilled nursing services to families with children who have intellectual and developmental disabilities (IDD).

Responsibilities

Conduct home visits with patients every 60 days.

Develop and implement individualized care plans.

Educate and support parents on their child's health needs.

Utilize telehealth to enhance communication and engagement with families and the care team.

Advocate for children's health and contribute to our mission of redefining care for the most vulnerable.

Requirements

Registered nurse with at least 2 years of experience and a current Colorado license.

Experience in home care, with a preference for those experienced in training family CNAs.

Comfortable working in a dynamic startup environment.

CPR certification required.

Pediatric Experience Preferred.

BLS certification required; PALS certification preferred.

Monday

Daily Schedule:

Mandatory nurse meeting in Denver office from 9am-10:30am.

The remainder of the day is dedicated to administrative tasks from home.

Tuesday-Friday

Conduct in-person recertifications (maximum 3 per day, typically 2).

Perform start-of-care assessments as needed (maximum 3 per week, usually 1 per day).

Remote work for charting, family communication, and case management.
