The Clinical Pharmacist Manager will lead efforts in providing care management services and support to vulnerable populations, while supervising a team of pharmacists and pharmacy technicians. Responsibilities β’ Provide telephonic and/or video and/or SMS-based outreach, engagement, and education of vulnerable patients in the Medicaid program in Illinois β’ Work with a multidisciplinary team to provide care management services that βwrap aroundβ primary care provider practices; team members include Community Health Workers, Care Coordinators, and Behavioral Health Therapists β’ Support patients and providers around transitions of care β’ Provide clinical services that support day-to-day operations and safe practices of the program β’ Participate in QI activities to assure quality, bolthires-effectiveness, and ROI β’ Utilize analytics dashboards that help you proactively outreach patients who receive medication and/or equipment rejections, admissions/readmissions, or who may benefit from medication adherence counseling β’ Lead and mentor pharmacists and/or pharmacy technicians Skills β’ PharmD with advanced practice license in good standing in Illinois.
Remote, located in Illinois. Chicago or central Illinois preferred β’ Completion of accredited PGY1 residency training in related discipline β’ 5+ years of experience in pharmacy practice including delivery of interventions/direct patient care in an ambulatory setting among vulnerable populations β’ Experience in multi-disciplinary care management β’ Positive referrals from prior colleagues β’ Completion of PGY2 residency training in related discipline (ambulatory care, geriatrics, pharmacotherapy, internal medicine) β’ Strong analytic skills and ability to navigate dashboards and registry data for proactive outreach campaigns including for HEDIS gaps in care β’ History of experience with medication management through collaborative practice agreements with primary care providers β’ BPS Board Certification Preferred (prefer BCPS and/or BCACP) β’ Experience serving vulnerable populations with a skill set including motivational interviewing, harm reduction, and behavioral health β’ Leadership experience supporting other pharmacists, pharmacy technicians and/or other care team members Benefits β’ Stock Options: Opportunity to invest in the companyβs growth.
