Job Summary:


We are seeking a compassionate and detail-oriented Clinical Support Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing exceptional support to our members, addressing their benefit questions, care coordination needs, and program inquiries. Key Responsibilities:



  • Respond to incoming calls from members, addressing their concerns, and providing timely support and guidance

  • Accurately document member interactions in our designated PHI system, ensuring seamless communication and continuity of care

  • Manage an assigned member caseload, providing short-term care coordination and health navigation to ensure optimal outcomes

  • Collaborate with our interdisciplinary care team to facilitate effective communication, identify potential risks, and improve quality and cost outcomes

  • Conduct outreach calls to members to confirm services and ensure their needs are being met

  • Perform additional duties as assigned, demonstrating flexibility and a commitment to delivering exceptional results

Requirements:



  • Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or work experience

  • High School diploma or equivalent

  • Proven ability to communicate effectively, both verbally and in writing, with excellent grammar, spelling, and punctuation skills

  • Experience working in a fast-paced environment, handling multiple priorities simultaneously

Preferred Qualifications:



  • Associate's Degree

  • Prior experience in a call center, care management, or healthcare environment, with familiarity with medical terminology and benefit products

  • Excellent telephone, organizational, and customer service skills

  • Team-oriented, organized, and detail-focused individual
