About the Role
We are seeking experienced, licensed physicians (MD/DO) and nurse practitioners (NP) to join our team on a contract basis to perform independent medical record reviews and author medical nexus opinion letters in support of veterans’ VA disability claims.
This is a fully remote, flexible contract position. You set your own schedule and accept cases based on your availability. Compensation is $200–$300 per completed letter/review, depending on case complexity.
- Responsibilities
- Conduct thorough, independent reviews of veterans’ medical records, service treatment records, and supporting documentation
- Author well-reasoned, evidence-based medical nexus opinion letters that address the “at least as likely as not” standard required by the VA
- Provide clear rationale linking the veteran’s current diagnosed condition(s) to their military service, including in-service events, injuries, exposures, or aggravation
- Ensure all opinions are supported by current medical literature and clinical reasoning
- Deliver completed letters within agreed-upon turnaround times (typically 5–10 business days)
- Communicate with our team regarding case questions, clarifications, or additional records needed
- Maintain strict HIPAA compliance and confidentiality of all veteran records and personal information
Must-Have Requirements
- The following requirements are non-negotiable. Please do not apply if you do not meet ALL of the criteria below:
- Proven experience writing medical nexus opinion letters for veterans’ VA disability claims — This is an absolute requirement. Candidates without direct nexus letter writing experience will not be considered.
- Active, unrestricted medical license (MD, DO, or NP) in at least one U.S. state — License must be in good standing with no disciplinary actions
- Board certification or eligibility in a relevant medical specialty (e.g., Internal Medicine, Psychiatry, Orthopedics, Neurology, Primary Care, etc.)
- Thorough understanding of the VA disability claims process, including the “at least as likely as not” (50% or greater probability) standard of proof
- Ability to review medical records and provide opinions grounded in evidence-based medicine and supported by current medical literature
- Strong medical writing skills — Letters must be professionally written, clearly organized, and legally defensible
- HIPAA compliance knowledge and commitment to maintaining the confidentiality and security of all veteran health information
- Reliable turnaround times — Must be able to consistently deliver completed reviews within 5–10 business days
- Preferred Qualifications
- Experience with BVA (Board of Veterans’ Appeals) hearings or IME (Independent Medical Examination) reports
- Familiarity with the 38 CFR (Code of Federal Regulations) and VA rating criteria
- Prior military service or experience working within the VA healthcare system
- Experience across multiple specialty areas (e.g., mental health, musculoskeletal, respiratory, cardiovascular)
- DEA registration (active)
- Malpractice insurance (active)
- Compensation & Details
- $200–$300 per completed nexus letter/medical review (based on case complexity)
- 1099 independent contractor position — this is not a W-2 employment role
- Fully remote — work from anywhere with a reliable internet connection

- Volume of cases available will depend on demand and your specialty coverage
Pay: $100.00 - $400.00 per hour
