Responsibilities • Responsible for case load/referral management and the overall activities of a specific Henry Ford at Home team • Utilizes the knowledge and application of nursing principles to ensure safe and effective coordination of patient care in their place of residence • Performs duties of the Home Health Care nurse Skills • Graduation from an accredited Registered Nurse program • Current licensure by the State of Michigan as a registered nurse • At least one (1) year of progressively responsible home health care nursing experience • Knowledge of computer use to electronically document patient care • Strong interpersonal and customer services skills to interact effectively with individuals from various cultural and socio-economic backgrounds • Current State of Michigan driver's license, an acceptable driving record, and reliable transportation • Ability to lift, carry, transport and move clients, if needed Company Overview • * Job seekers: please be aware of fraudulent job postings and phishing scams via LinkedIn.
