We are seeking an experienced RN to serve as our Admissions & MDS Coordinator — a pivotal leadership role responsible for guiding resident admissions while ensuring accurate assessments, regulatory compliance, and reimbursement integrity within our Skilled Nursing Facility. About the Role This unique dual-role position blends clinical leadership, care coordination, and admissions oversight. As our RN Admissions & MDS Coordinator, you will dedicate approximately 50% of your time to leading and coordinating admissions into our Skilled Nursing Facility and 50% to overseeing the MDS process. You’ll serve as a key clinical resource, ensuring regulatory compliance, accurate reimbursement, seamless resident transitions, and exceptional first impressions for new residents and families. What You’ll Do: Admissions & Care Transitions (50%) Coordinate and oversee all clinical aspects of admissions into the Skilled Nursing Facility. Review referral information and conduct clinical evaluations to ensure appropriate placement. Collaborate with hospital discharge planners, providers, and internal teams to facilitate smooth transitions. Communicate clearly and compassionately with residents and families throughout the admission process. Ensure timely, accurate, and complete admission documentation within the EMR. Partner with leadership to support census development and occupancy goals. MDS & Clinical Reimbursement (50%) Oversee and complete MDS assessments for Transitional Care (TCU) and Long-Term Care (LTC) residents. Ensure accurate, timely, and compliant coding in accordance with federal and state regulations. Audit documentation to validate diagnoses and support appropriate reimbursement under the Patient Driven Payment Model (PDPM). Collaborate with interdisciplinary team members to develop and maintain comprehensive care plans. Monitor regulatory updates and maintain survey readiness. What You Bring: Current RN license in the state of Montana (or compact equivalent) — required. Strong working knowledge of MDS 3.0, PDPM, and regulatory requirements in skilled nursing. Experience in admissions, case management, or care coordination preferred. Resident Assessment Coordinator-Certified (RAC-CT) credential preferred. Proficiency in EMR systems, including PointClickCare (PCC) and Meditech. Excellent organizational, communication, and critical-thinking skills. Ability to balance detail-oriented documentation with relationship-focused care coordination.
