Schedule: Monday-Friday, 9:00 AM - 6:00 / 10:00 AM - 7:00 PM (flex). To participate, you must be enrolled in an Amedisys medical plan. You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Responsibilities β€’ Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs. β€’ Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team. β€’ Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician. β€’ Functions as a liaison between disciplines and facilities during the admission process. β€’ Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate. β€’ Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required. β€’ Performs on-call responsibilities and on-call services to patients/families as assigned. β€’ Performs other duties as assigned. Requirements β€’ Current RN license, specific to the state(s) you are assigned to work. β€’ 1+ years of clinical RN experience in acute healthcare setting(s) β€’ Current CPR certification. β€’ Valid driver's license, reliable transportation, and liability insurance. β€’ Hospice or home health experience admission or case management experience. Nice-to-haves β€’ Strong communication and interpersonal skills β€’ Ability to work independently and manage multiple priorities β€’ Comfortable with daily travel between facilities β€’ Proficient in electronic medical record (EMR) systems, HCHB exp.
