Additional Requirements and Day-to-Day Tasks of Position
The Bureau of Social Care and Community Supports works to integrate health and human services within the Medicaid program. The bureau is composed of two units: Housing Unit and Social Determinants of Health (SDH) Unit. This position is in the Social Determinants of Health unit and will support the implementation of health-related social needs programs within the Medicaid program. The selected candidate will have the unique opportunity to support the Social Care Network (SCN) program implementation under New York’s 1115 Waiver Amendment.
Tasks include:

Act as a liaison between the Social Care Networks and other stakeholders. This collaborative aspect of the role underscores the importance of your position, as you will be the bridge that ensures effective communication and coordination, making you an integral part of the team.
Provide technical assistance, contract management, and performance monitoring of Social Care Networks. One of the key responsibilities of this role is the fiscal administration of grant funds, including oversight of work plan deliverables, budget expenditure, and narrative reports. This aspect of the role requires strong communication, data analytics, and performance management skills.
Engage Regional Social Care Network (SCN) lead entities, community-based organizations (CBOs), and stakeholder coalition groups in each SCN region in regular planning convenings, workgroup meetings with the goal of understanding needs and mapping out regional/community priorities.

Support the evaluation of the Social Care Network activities and overall NYHER 1115 Waiver implementation.

Create a broad range of written material, including policies and procedures for internal & external stakeholders.

Research policy areas relevant to Division projects, programs, and activities. Elicit requirements from subject matter experts in the context of projects, initiatives, and process improvements.

Develop presentations, facilitates large and small group meets and collaborative events.
This person will be expected to work in the office one day per week on Wednesdays for in-person office meetings

Basic Qualifications:

8 years of professional experience
Bachelor’s degree is highly desired.
Experience in social care and/or healthcare services required.
Must have solid writing and verbal communication skills and general familiarity with Windows operating system and common software, such as Microsoft Office suite and Adobe Pro

Regional Salary Ranges:
Albany, NY: $62,353.20 - $76,910.00
NYC: $68,588.52 - $84,600.00
