About the position
- Responsibilities
- Partner with a registered nurse case manager to assess and plan for members' health needs.
- Research available options and resources for members with financial, transportation, safety, and housing needs.
- Empower members to make positive lifestyle and medical choices.
- Assist members with community resources for medical, financial, and behavioral health.
- Conduct assessments and provide support to a diverse population throughout their lifecycle.
- Engage in telephonic and electronic communication with members.
- Requirements
- Must have a Master's Degree in Social Work (MSW) from an accredited program.
- Experience in healthcare social work or a related field is preferred.
- Strong communication skills, both verbal and written, to effectively interact with members and clinical staff.
- Ability to work independently and manage time effectively in a telework environment.
