Responsibilities β€’ Assesses physical, environmental, and emotional factors telephonically to determine hospice needs. β€’ Utilizes EHR, including the patient's plan of care to develop recommendations. β€’ Provides recommendations, patient/caregiver education/instructions and hospice support telephonically based on the situation and the plan of care.

    β€’ Collaborates with pharmacies, DME vendors and other agencies for effective patient management. β€’ Facilitates delivery or maintenance of provided medical equipment to meet patient needs. β€’ Assigns all visits, admissions and follow-up calls to on-call field staff (RN, LPN, HA, CH, SW) as needed. β€’ Submits accurate and detailed documentation in real-time to promote continuity of care. β€’ Utilizes a combination of agency resources and nationally recognized standards of practice to achieve excellent pain and symptom management and high-quality end-of-life care.

    β€’ Participates in agency performance improvement initiatives. β€’ Performs other duties as assigned. Qualifications β€’ Current, unencumbered license to practice as a registered nurse in the state you are assigned to work. β€’ One year of experience as a registered nurse. β€’ Current CPR certification. β€’ Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers. β€’ Must be comfortable with technology. β€’ Must be willing and eligible to obtain additional RN licenses in other states (reimbursed).
