The job is a remote job and is open to candidates in USA. Carle Health is a healthcare system based in Urbana, IL, and they are seeking a Home Care Scheduling Coordinator. The role involves centralized scheduling for nursing, therapy, and home health aides, focusing on optimizing staff productivity and ensuring efficient patient care. Responsibilities ā¢ Answers phones in accordance with customer service standards and directs calls to appropriate person ā¢ Maintains centralized scheduling function for Nursing, Therapy and Home Health Aide visits ā¢ Assigns admission visit, primary nurse and daily visits based on most effective and efficient use of staff resources; considers future need of patient, skill of nurse, geographic locations of staff and patient ā¢ Monitors staff productivity and overtime hours; ensures staff is scheduled according to standard and overtime is kept to a minimum ā¢ Maintains ongoing working (draft) schedule for current plus one week for use as worksheet to determine staff availability for assignments ā¢ Keeps clinical manager and case managers advised of daily schedule needs.
Notifies manager of clinical hours that can be reduced due to low scheduled patient visit for following day ā¢ Coordinates closely with referral intake, advising of availability for new admissions for next 2-3 days to ensure smooth transition into Home Care ā¢ Monitors all discharge and cancellation communications and removes unneeded visits from future schedules ā¢ Maintains a log of scheduling changes as called in by Home Care staff. Log includes date/time of each call, caller, scheduling change request and change that was made.
Notifies management and staff of changes made to schedules ā¢ Assist leadership team with staff schedule to facilitate productivity, mileage stewardship, and reduce in overtime ā¢ Completes monthly staff schedule for nursing including on-call assignment. Enters on-call assignment into system for home care and hospice nursing Skills ā¢ 2+ years of customer service experience ā¢ Strong computer skills ā¢ Interpersonal skills ā¢ Ability to successfully work with demanding personalities and meet internal/external customer needs without becoming involved emotionally ā¢ Able to work independently to prioritize visits ā¢ Determine effective routing to optimize the time of professional staff while minimizing travel time and distance ā¢ Perform centralized scheduling for nursing, therapist and aides ā¢ Ensure continuity of communication, documentation, and required record keeping ā¢ Ensure work load and staff productivity are met ā¢ Open communication with Home Care Manager Benefits ā¢ Comprehensive benefits package Company Overview ā¢ Carle Health is a vertically integrated system with a bold but simple mission: to be the trusted partner in all healthcare decisions for everyone who depends on it.
