They are seeking a Case Management Coordinator who will assess, plan, implement, and coordinate case management activities for members, focusing on enhancing their overall wellness through integrated services. Responsibilities Assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs Managing a caseload that includes supportive and medically complex members Developing a proactive course of action to address issues presented to enhance short and long-term outcomes Determining appropriate services and supports due to members' health needs, including prior authorizations, coordination with PCP and skilled providers, condition management information, medication review, and community resources Skills Must reside in Melbourne FL, or surrounding areas such as Palm Bay and Satellite Beach FL Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet members face to face in Melbourne and surrounding areas Palm Bay and Satellite Beach.
