The role involves providing exceptional physical therapy services in patients' homes, focusing on improving mobility and quality of life through personalized care. Responsibilities Patient Assessment: Conduct comprehensive evaluations of patients' physical conditions, mobility, strength, balance, and functional abilities in their home environment Treatment Planning: Develop individualized treatment plans with measurable goals focused on improving mobility, reducing pain, and restoring functional independence Therapeutic Interventions: Implement evidence-based physical therapy techniques including therapeutic exercises, manual therapy, gait training, and balance activities Patient & Caregiver
Education: Provide expert guidance on home exercise programs, fall prevention strategies, proper use of assistive devices, and safe mobility techniques Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments Care Coordination: Collaborate effectively with physicians, nurses, other therapists, and healthcare team members to ensure comprehensive patient care Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements Safety Assessments: Evaluate home environments for fall risks and recommend modifications to enhance patient safety Skills Graduate of an accredited Physical Therapy program (DPT or equivalent) Valid Illinois Physical Therapist license Current CPR certification Valid driver's license and auto insurance Open to new graduates and experienced physical therapists - comprehensive training provided Willingness to learn WellSky Kinnser EMR system (training included) Strong clinical assessment and critical thinking abilities Outstanding written and verbal communication skills Patient-centered care philosophy with cultural sensitivity Strong organizational and time management capabilities Passion for community service and home-based healthcare Authorization to work in the United States or eligibility for H1B sponsorship English proficiency equivalent to successful therapy practice in the U.S Commitment to long-term employment with Safe Life Home Health Care No prior home health experience necessary, but preferred Benefits Premium health, dental, and vision insurance coverage Flexible spending accounts (FSA/HSA) Life and disability insurance options
