Healthkeeperz is hiring a CAP Case Manager to support the Community Alternatives Program (CAP) for Disabled Adults and Children. This role provides assessment, care planning, coordination, and ongoing monitoring to help individuals safely remain and receive care in their homes. Responsibilities Conduct assessments and determine CAP eligibility Develop individualized care plans based on identified needs Coordinate services with families, providers, and community agencies Perform home visits and follow-up monitoring Maintain accurate documentation in E-CAP; review aide notes; support compliant billing Ensure compliance with CAP and state program guidelines Participate in required training and continuing education Skills Bachelor's in Social Work + 1 year related experience Bachelor's in Human Services + 2 years related experience Bachelor's in Non-Human Services + 2 years related experience Current NC RN license + 1 year case management experience Apprentice/Trainee (Bachelor's or RN without full experience) – requires ongoing supervision Knowledge of casework, care coordination, and social services practices Familiarity with community resources Strong communication, time management, and organization Ability to travel for visits and meetings Proficiency with electronic documentation and virtual communication tools Benefits Mileage reimbursement $40 monthly mobile phone stipend 401(k) with employer match Medical, dental, vision Life insurance; Employee Assistance Program PTO + 9 paid holidays Company Overview HealthKeeperz provides integrated home-based medical care, equipment and services.
