The Case Management Specialist plays a vital role in the arenaflex team, utilizing critical thinking and judgment to facilitate the case management process. As a Case Management Specialist, you will work closely with members, providers, and community associations to identify and address the unique needs of each individual. Your primary goal will be to provide exceptional support and guidance, ensuring that members receive the best possible care and services.
By collaborating with cross-functional teams, you will help to develop and implement personalized care plans, addressing the full continuum of members' healthcare and social determinant needs. Key Responsibilities Conduct comprehensive assessments of members' needs and eligibility using care management tools and data analytics, and recommend approaches to achieve case resolution or address needs. Identify high-risk factors and service needs that may impact member outcomes and care planning, and refer cases to clinical case management or crisis intervention as necessary.
Coordinate and implement assigned care plan activities, monitoring progress and adjusting plans as needed. Enhance clinical appropriateness and quality of care by collaborating with case managers, supervisors, Clinical Directors, or other healthcare programs to overcome barriers to meeting objectives. Present cases at case conferences to obtain multidisciplinary reviews and achieve optimal results. Identify and escalate quality of care issues through established channels. Utilize communication skills to secure appropriate decisions and services necessary to meet members' benefits and healthcare needs.
Employ influencing/persuasive communication skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimal health. Provide coaching, information, and support to enable members to make informed, independent clinical and healthy lifestyle decisions. Assist members in effectively and knowledgeably participating with their providers in healthcare navigation. Monitor, assess, and document care using case management and quality management processes in compliance with regulatory and licensure requirements and company policies and procedures.
Essential Qualifications To be successful in this role, you will need to possess the following essential qualifications: 3+ years of experience in a health-related field. Bachelor's Degree or equivalent experience. Preferred Qualifications
The following qualifications are preferred, but not required: CRC, CDMS, CRRN, COHN, or CCM certification. Medicare and Medicaid experience. Managed care insight. Experience working with geriatric special needs, social health, and disabled populations. Knowledge of assessment, screenings, and care planning.
Bilingual (English/Spanish; English/Creole).
