The virtual medical assistant plays an important role in the management of the chronic conditions for patients for Primary Care Practices to include Gastroenterology, Urology, Cardiology, Pain Management. Working in conjunction with the care team and physician to support and assist patients in obtaining the resources they need to improve upon their health, happiness, and longevity. Description As a virtual medical assistant at Complete Care Partners, LLC, you will play a crucial role in supporting and assisting patients in managing their chronic conditions.

You will work closely with the care team and physicians to ensure that patients have access to the resources they need to improve their health outcomes. Responsibilities: β€’ Collaborate with the care team and physicians to develop and implement care plans for patients β€’ Coordinate appointments, referrals, and follow-up care for patients β€’ Assist patients in navigating the healthcare system and accessing necessary resources β€’ Monitor patient progress and communicate any changes or concerns to the care team β€’ Maintain accurate and up-to-date patient records and documentation β€’ Provide education and support to patients and their families on managing their health conditions Requirements: β€’ High school diploma or equivalent β€’ Certified Medical Assistant β€’ Previous experience in healthcare coordination or a related field is preferred β€’ Strong communication and interpersonal skills β€’ Ability to work effectively in a fast-paced environment β€’ Knowledge of medical terminology and healthcare systems β€’ Ability to maintain confidentiality and adhere to privacy regulations Please note that Complete Care Partners, LLC is an equal opportunity employer and does not discriminate based on race, color, religion, sex, national origin, age, disability, or any other protected status.
