Role Description We are seeking a detail-oriented and compassionate Virtual Care Coordinator to remotely oversee client care plans, facilitate caregiver assignments, and act as a liaison between clients, families, and care teams. The ideal candidate will ensure high-quality home care services that promote client dignity, independence, and wellness — all while maintaining compliance with U.S.
home care regulations. • Conduct remote client intake assessments via phone or video, developing personalized care plans in collaboration with families and care staff. • Monitor client satisfaction and address evolving care needs, adjusting service plans as necessary. • Maintain regular virtual check-ins with clients and families to ensure services meet expectations. • Use scheduling platforms (e.g., HHAeXchange) to coordinate caregiver visits and assignments. • Respond promptly to schedule changes, call-outs, and emergencies to ensure consistent service coverage.
• Collaborate with HR and operations teams to assign qualified caregivers based on availability, location, and client requirements. • Maintain electronic records and documentation in line with HIPAA and relevant home care regulations. • Utilize Electronic Visit Verification (EVV) tools to monitor service delivery. • Ensure all service plans and documentation are up-to-date, accurate, and securely stored. • Serve as the remote point of contact for clients, families, caregivers, and internal teams. • Escalate concerns, compliance issues, or incidents to appropriate internal departments.
• Participate in virtual team meetings via bolthires Meet, Chat, or similar platforms. • Review and evaluate the quality and effectiveness of virtual and in-home care services. • Follow up on client feedback, resolve concerns efficiently, and document all outcomes. • Support periodic service audits and performance reviews to maintain high operational standards. Qualifications • Bachelor’s degree in Healthcare, Social Work, Human Services, or a related field (preferred). • Minimum 2 years of experience in care coordination, home care, or healthcare administration.
• Proficiency with scheduling and client management platforms (e.g., HHAeXchange, Basecamp, Promise Portal). • Strong understanding of U.S. home care compliance standards (Chapter 611 or equivalent). • Excellent written and verbal communication skills. • Ability to manage multiple priorities independently in a remote setting. • Stable internet connection and a quiet, professional home workspace.
