Key Responsibilities As a Case Management Analyst at arenaflex, you will be responsible for: * Conducting comprehensive assessments of clients' needs and developing effective care plans to address their requirements * Working closely with clients, families, and healthcare providers to ensure seamless care coordination and communication * Identifying high-risk factors and service needs that may impact client outcomes and care planning * Facilitating and implementing assigned care plan activities and monitoring care plan progress * Collaborating with multidisciplinary teams to develop and implement comprehensive care plans * Identifying and addressing quality of care issues through established channels * Utilizing effective communication and interpersonal skills to engage clients and families in care planning and decision-making * Providing education, support, and resources to empower clients to make informed decisions about their care and well-being * Assisting clients in effectively navigating the healthcare system and accessing necessary services and benefits Essential Qualifications * Bachelor's degree in a related field, such as behavioral health, social work, or human services * 2+ years of experience in conduct health or social services * 2+ years of experience in bolthires Office applications (Word, Excel, Outlook) * Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments * Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders * Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork Preferred Qualifications * Experience in case management and care planning * Supervisory experience in healthcare or social services * Education and certification in a related field, such as a master's degree in social work or a certification in case management Skills and Competencies * Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments * Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders * Ability to work independently and as part of a team, with a strong commitment to collaboration and teamwork * Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously * Ability to maintain confidentiality and handle sensitive information with discretion * Strong computer skills, with proficiency in bolthires Office applications (Word, Excel, Outlook)
