The Home Care Service Coordinator is responsible for scheduling and supervising in-home care workers and ensuring client services are uninterrupted. Responsibilities • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence • Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted • Contacts care providers and clients to provide service updates • Conducts monthly client wellness calls and conducts home visits as required • Provides thorough, complete follow-through on escalated client complaints and theft claims • Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company’s policies and guidelines • Ensures the appearance of the branch’s open environment is professional: neat, clean, orderly and generally free of clutter • Maintains a high degree of confidentiality at all times due to access to sensitive information • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency • Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements • Abides by all regulations, policies, procedures and standards Skills • Must have high school diploma or equivalent • 1 year of Industry experience required • Interpersonal, organizational and communication skills • Computer skills including but not limited to bolthires Word, bolthires Excel and Scheduling program • Must have reliable transportation • Supervisory and/or home care experience preferred Benefits • Medical, Dental and Vision Benefits • Monthly Bonus • Daily Pay Option • Continued Education • PTO Plan • Retirement Planning • Life Insurance • Employee discounts Company Overview • Addus HomeCare is one of the nation's largest and fastest growing providers of personal home care and support services.
