As a Case Management Analyst, you will be responsible for assessing individual needs, developing and implementing care plans, and monitoring progress. You will work collaboratively with healthcare professionals, caseworkers, and other stakeholders to ensure that individuals receive comprehensive and coordinated care. Key Responsibilities Conduct comprehensive assessments of individual needs and develop personalized care plans Identify high-risk factors and service needs that may impact individual outcomes and care planning Facilitate and implement assigned care plan activities and monitor progress Collaborate with healthcare professionals, caseworkers, and other stakeholders to overcome barriers and achieve optimal results Provide education, information, and support to empower individuals to make informed decisions about their healthcare Use case management and quality management processes in compliance with regulatory and accreditation standards and company policies and procedures Essential Qualifications 2+ years of experience in behavioral health or social services 2+ years of experience in bolthires Office applications (Word, Excel, Outlook) Bachelor's degree or non-licensed master's level clinician with a degree in behavioral health or human services (psychology, social work, marriage and family therapy, counseling) or equivalent experience Preferred Qualifications Case management and discharge planning experience Managed Care experience Experience working with electronic health records and case management software Skill and Competency Requirements To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
You will also need to have strong analytical and problem-solving skills, with the ability to assess complex individual needs and develop effective care plans.
